Tormented Souls is a game that pays homage to classic style Survival Horror games, taking inspiration from franchises such as Resident Evil, Silent Hill and Alone in the Dark. Regarding the Roadmap we are talking about a minimum of two playthroughs, a first playthrough to focus on collectibles, combat-related trophies and getting the best "Adoption" ending, then a second playthrough, in less than 3 hours, without healing and without saving, while obtaining the worst ending "Remorse".

The game is focused on puzzles and progression so you will get most of the trophies as you progress through the game; however, if at any point you feel lost and don't know where to go, you can use the video guide in Anxiety trophy_gold.png or, if you prefer, check a text walkthrough (credit to PowerPyx for this): LINK. Finally, if you need information about how to solve the various puzzles in the game or are simply looking for strategies to eliminate each enemy, you can check the Additional Resources Suplementary Guide I created.

Step 1: Complete the Game with the "Good" Ending and Work on Miscellaneous & Collectibles

At this stage we will focus on beating the game by obtaining the best ending "Adoption" (which will require you to perform an optional in-game sidequest) and getting all the miscellaneous and collectible trophies.

Use this stage to learn the mechanics of the game and give yourself an idea of the enemies and rooms you will encounter, as well as the items you will be obtaining (in short, enjoy this return to survival horror!).

Finally, it is important to mention that, although suggested for learning more about the story, you do not need to obtain all the files in the game, only the 32 related to the 4 sets of diaries. All of them are in plain sight so chances are you will get most (if not all) of them just by playing the game and exploring each room.

At the end of this step you will have earned the following trophies:

Step 2: No Save/No Healing/Speedrun in less than 3 Hours and "Bad" Ending

In this stage we will use everything learned during the first playthrough, to obtain the last trophies, performing a Speedrun in less than 3 hours, without saving, without healing a single time and while getting the "Remorse" ending. While this would seem difficult to achieve during your first playthrough, the game is quite short and the enemies in most cases follow patterns that are very easy to identify. Likewise, there is more than enough ammunition and healing items (morphines and first aid kits) to approach this stage without too much troubles.

However, if you prefer to take it a little more calmly, you can use this stage to do a speedrun in less than three hours without saving and leave your non-healing playthrough for an -optional- additional stage.

At the end of this step you will have earned the following trophies:

Untouchable trophy_gold.png - Missable
Anxiety trophy_gold.png - Missable
Amnesia trophy_gold.png - Missable
Remorse trophy_silver.png

Step 3: (Optional) No Healing Playthrough

If you chose to leave your non-healing playthrough for this stage, do so now and conclude with your adventure at Wildberger Hospital!

At the end of this step you will have earned the following trophy:

  • Complete the game without using any health items

    POTENTIALLY MISSABLE

To get this trophy you must beat the game without healing once, which is not to say that you cannot take any damage! The way the damage works in this game is that, depending on the enemy, normally two attacks (barring strong hits from Hammers or, for example, long grabs from the Ghost) will change your life from "Fine" to "Caution" and two additional attacks will land you in "Danger". Although you can play normally with your energy in Caution, entering Danger will imply a reduction in Caroline's speed and therefore she will be more vulnerable to attacks.

    Some recommendations for this run:
    • Save frequently after having covered a good portion without being harmed. There are 14 Recording Tapes in the game, most of which are in rooms that you must visit in order to progress through the game so make good use of them. For a game that could take a around two hours once you know what to do, this implies that you can save every 8 to 10 minutes of game time so it is quite doable! Some recommended saving points are as follows:
      1. In the first saving room you encounter (E-1F Examination Room). This is due to the two annoying Crawlers you will find and that could bring you some troubles .
      2. Before using the Thread Bolt to go to the Observation Room (W-2F West Wing Corridor) and face the first mini-boss.
      3. (W-1F Studio) Right after defeating the first miniboss and before using the Scalpel to descend the stairs from the Service Area.
      4. (W-2F Room 2E), after (or before) solving the mannequin puzzle and obtaining the Electric Lamp.
      5. (E-1F Delivery Room), the first time you arrive at this place.
      6. Sewer Storage Room. After solving the Moon and Sun Necklace puzzle.
      7. Bunker 2C, after obtaining the second Pipe Cap.
      8. Graveyard, before the final boss.
    • Reload your weapons from inventory to save time and prevent enemies from attacking you during the reload animation; at the same time, count your shots! The Nail Gun magazine can carry up to 10 nails, while the Shotgun magazine can carry only one shell.
    • Plan your route in advance. It is worth knowing which enemies will come in each room and analyze the best way to deal with them, either by eliminating them or escaping. Check out the Additional Resources Suplementary Guide for more information on how to deal with each enemy.
    • Building on the above, follow a Video Guide! You can use the video guide included Anxiety trophy_gold.png in order to beat the game with No Saves and No Healings in less than 3 hours (1:35:27 to be more precisely) and thus save an additional run or if you prefer, you can check the following No Damage/No Healing in 1:31:22 video guide (not too much difference in terms of length) in which the (7) recommended saving opportunities are presented and move forward with a little more calm.
  • Let the shadows consume you

    POTENTIALLY MISSABLE

At any time during the game, if you stay for a long time in completely dark spaces, without turning on your Lighter or (later) using the Electrical Lamp, you will start to see colored lines and quickly the game will run a video in which you will die tragically.

The first chance you will have to get this trophy is as soon as you leave the Private Bathroom at the beginning of the game, for this, instead of picking up the lighter at the end of the corridor on your right, take the left path towards the dark corridor. As soon as the animation ends, the trophy will be yours.
  • Travel to the past

You will get this trophy the first time you travel to the past by using the VHS Tape (Experiment), found inside the open cell of the Experiment Room (hidden room behind a cabinet in the Basement's Morgue), on the projector located in the E-2F Meeting Room.
  • Solve all the combination key puzzles.

You will get this trophy once you solve the 4 combination key puzzles in the game. To do this, you must first find the Incomplete Key inside the cell in which Anna is imprisoned by using the Acid in the lock during your encounter with her in the Experiment Room of the past and returning to this same place in the present after doing so. The second necessary item is the Metal Dial, which is found in the open bathroom area of the Basement (this is the place near where you find the electrocuted water).

Once you have both items, combine them and proceed to solve the four door puzzles (Check out the Additional Resources Suplementary Guide for the solutions of the puzzles within the game).
  • Escape from the bathroom

As soon as you start the game, go to the cabinet with the padlock and use the code (from top to bottom) 1-2-3-5 to get the wrench, adjust it once and use it on the door to escape from the Private Bathroom. As soon as you do so, you will get the trophy.
  • Loop

    Escape the infinite room

    POTENTIALLY MISSABLE

The Infinite Corridor is an optional room located at the beginning of the Mausoleum. To get to it, as soon as you solve the Key Dial puzzle and enter the door that connects the Main Hall to the Mausoleum, go down the two sets of stairs and, instead of going straight through the door, turn right at the first at the first intersection. You will hear a Slider approaching and, behind it, you will see a series of warning signs, indicating that you have arrived at the right place.

Once inside the room, you must solve the short puzzle by entering the 7 doors in a specific order: Golden door, Empty basin door, North door, Wooden door, Earth basin door, Ember basin door and West ("Oeste") door. If you did well, a candle will light for each successful attempt, otherwise all will all go out and you will have to start over. As soon as you leave the room, you will get the trophy and you will be able to access Bertram's Office, where you can find the Antidote necessary for the  Sisters trophy.

If you prefer a visual guide on how to escape from the infinite corridor, you can watch the following video:
  • Complete the game in under three hours

    (This tophy is potentially missable)

Completing the game in less than three hours might seem difficult after your first playthrough, which if you didn't follow a guide, could have taken you around 10 hours; however, once you know where to go and what to do with each puzzle, time is greatly reduced and you may be able to beat the game even in less than two hours without getting too desperate. Having said that, it is very important to mention is that the timer continues even though you pause the game so try not to take too long pauses.

Likewise, it is highly recommended that you combine this trophy with Amnesia trophy_gold.png or, even better, if you dare to do so, with both Amnesia trophy_gold.png  and Untouchable trophy_gold.png so that you can save yourself an additional run.

Finally, you can follow the video guide provided below, which shows how you can beat the game in less than three hours (1:35:27), without healing and without saving your progress!
  • Kill an enemy using a trap

    POTENTIALLY MISSABLE

There are two types of traps in the game, the Armored Knights in the Hospital and the Spike Traps in the Bunker and to get this trophy, you will need to kill an enemy with one of these traps.

The first place you can do this (and potentially the easiest) is at the E-2F Dining Room Hallway, in this corridor there is a Wheelchair close to the Main Hall exit and a set of armored knights by the door that overlooks the Library. For the trophy, just lure the Wheelchair by walking slowly as you get close to the armored knights and when it gets close, run past the trap activating it along the way. If you got it right, the Wheelchair will pass through as soon as their lower their swords and will die in one hit.

If you prefer a visual guide, you can watch the following video:
  • Complete the game without saving

    POTENTIALLY MISSABLE

It is recommended to combine this trophy with Anxiety trophy_gold.png and in order to get it, you will have to beat the game without saving a single time.

While it might seem complicated, considering that six basic attacks from enemies can end Caroline's life), there are quite a few Morphines (more than 20) throughout the game and along with the 7 First Aids Kids that you can get, will make this journey a little more bearable. Anyway, you will have to be much more careful with the enemies (especially during the battle with the final boss) so it does not hurt to plan your way in advance.

For this, you can follow the video guide provided in the Anxiety trophy_gold.png walkthrough which shows how you can beat the game in less than three hours (1:35:27), without healing and without saving your progress!
  • Save the game

    POTENTIALLY MISSABLE

You can save your progress by using Recording Tapes on recorders scattered in "safe rooms" throughout the game. The first chance you will have to obtain this trophy is in the E-1F Examination Room, right after exiting the Private Bathroom and obtaining the lighter.

Although difficult during the first playthrough, this trophy is technically missable if you consider that you can get through the game without saving once.
  • Create the shotgun

    POTENTIALLY MISSABLE

Unlike other survival horrors, this time you will need to "create" the Shotgun by combining the two parts you find in the game. Something positive about this is that both parts are hidden in plain sight (and in your path) and you will be able to tell their location by the baby dolls that hold them.

The location of the Shogun parts is presented below:
    • Shotgun Part A: (W-1F Service Area), in the corridor that connects the Studio with the Main Hall.
    • Shotgun Part B: (W-2F Observation Room), In the room where you meet the Priest for the third time, after removing the Thread Bolt from the Reception and using it in the West Wing Corridor.
  • Pick up five first aid kits

    POTENTIALLY MISSABLE

To get this trophy, you will need to obtain at least 5 of the 7 hidden first aid kits hidden n the game. While this trophy is technically missable, it is very likely that you will get all of them by investigating thoroughly each room you enter.

The location of the 7 first aids kits is preset below:
    1. (W-F2 Room 2E), inside a medical cabinet inside the save room.
    2. (Basement Boiler Room), inside a medical cabinet, after you deal with a Crawler.
    3. (E-F1 Pharmacy), inside a medical cabinet, after solving the combination key door puzzle.
    4. (W-F2 Room 2A), in the optional room where you get the Electrical Lance weapon,
    5. (W-F1 Shower Room), inside on a medical cabinet behind the bathbub with the jump scare.
    6. (Bunker 3A), inside a medical cabinet, after getting the Gas Canister from the Prison and dealing with the two Slashers.
    7. (Slaughter Room), on a table during the final boss fight.
  • Save only yourself

In order to obtain this trophy you will need to beat the game while obtaining the "bad" ending "Remorse".

To achieve this ending you must avoid interacting with the Ghost that is crucified inside the Preparation Room, this is the room where you face four Atomic Men "Hammers". After the battle, simply use the elevator to proceed to the Graveyard and defeated the final boss as you will normally do. Once you do so, "escape" from the Hospital by using the Cross Key on the front door.
  • Start the generator

After your first encounter with the Priest, you will have to activate the Hospital's electricity generator by solving the first puzzle of the game. After restoring the power, you will get the trophy.

Check the Additional Resources Suplementary Guide for more information on how to solve all the puzzles in the game.
  • Start the pump

In order to get this trophy, you must first solve the Gasoline Pump Puzzle inside Bunker Room 1-2B (check the Additional Resources Suplementary Guide section for more information on how to solve this puzzle), for which you must frist obtain the 3 Pipe Caps located in various parts of the Bunker (Room 1A, Room 2A and Room 3B), as well as the Gas Canister of the Prison and use both of them to fill the Gas in the canister.

Once full, proceed to the lower level of the Bunker, fill the engine with gasoline and pump the water three times to get this trophy.
  • Unlock the door under the hall stairs

The door under the hall stairs is the door that connects the Main Hall of the Hospital with the Mausoleum and to open it you will need to obtain three key items and solve the Key Dial Puzzle with the last of them.

The location of these three items is presented below:
    • Frozen Blood Medallion:  To create this item, you will first need to get the Empty Mold from the W-F1 Laundry, then solve the Cash Register Puzzle to obtain the Coin. Use this Coin on the vending machine inside the Chapel and get the Blood Bag. Combine both items to create the Filled Mold key item and travel to the past through the VHS Tape (Chapel) in order to store the Mold inside the Kitchen's freezer, then, return to the present to otain the Frozen Blood Medallion from the same place.
    • ID Card: Found at the end of the tunnel in E-F2 Pumping Room, after using the Jade Eye obtained from the Grandfather Clock Puzzle in the Office and solving the Vinyl Disc Puzzle.
    • Key Dial: Located in E-2F Library, after inserting the charged Battery into the forklift to reach the second floor. Once here, deal with the two Sliders  and look for a chessboard on which you will find the dial.
Once you have all three items, proceed to the mechanism under the stairs in the Main Hall, insert the Frozen Blood Medallion and the ID Card and solve the Key Dial Puzzle according to the pattern shown in the image below. Once you insert the Key Dial with the correct pattern, the trophy will be yours.

  • Kill 40 enemies

    POTENTIALLY MISSABLE

Unless you are consciously avoiding killing monsters, you will get this trophy shortly before entering the Mausoleum.

Although it is technically possible that you could miss this trophy, the game balances the amount of ammunition at your disposal quite well so it is possible to even kill all the enemies of the game (a little over 70 enemies). In this way and so that you do not have problems in obtaining this trophy, simply try to eliminate all the enemies you meet until you get it, considering the recommended strategies to deal with each type of them mentioned in the Additional Resources Suplementary Guide.
  • Kill 20 enemies

    POTENTIALLY MISSABLE

Check Serial Killer trophy_gold.png for more information.

Secret trophies

  • Escape with Anna

    POTENTIALLY MISSABLE

In order to obtain this trophy you will need to beat the game while obtaining the best ending "Adoption".

To achieve this ending you must obtain two important items: the first, the VHS Tape (experiment), is obtained from the only open cell in the Experiment Room (hidden room in the Morgue of the Hospital's Basement) and is Key Item necessary to progress with the story; and the second, the Antidote, which is found in Bunker 3B, next room after exiting from the Infinite Corridor (check Loop trophy_silver.png   for more information), guarded by two Slashers and a Waller.

Once you have these items (and not before) proceed to the Preparation Room, this is the room where you face four Hammers at the same time and that serves as the last miniboss in the game, as well as the point of no return. As soon as you defeat the Hammers, use the Antidote on the Ghost that is crucified (do it before going down in the elevator) and you will get the Bolt Cutters. Later and once you have defeated the final boss, return to the Meeting Room and use the VHS Tape (experiment) again to return to the past, go to the cells and use the Bolt Cutters to free Anna from her cell. Finally, escape from the Hospital by using the Cross Key on the front door.
  • Find your old hiding place

After obtaining the Sun Necklace, by solving the TV Dial Puzzle (check the Additional Resources Suplementary Guide section for more information on how to solve this puzzle) in Caroline's room and getting the Moon Necklace from the safe inside the Sewer Storage Room, return through the E-2F Office to the Children's Bedroom and use the combined necklace on the plaque of the wall next to the bed. This will open a secret room, which after approaching to it, will get you the trophy.
  • Open the vault

After obtaining Remembrance trophy_bronze.png and collecting the Fishing Hook from this hidden room, return to the past through the new VHS Tape (Dentist)  that spawns in the E-2F Office to acquire the Eye. Combine this item with the fishing hook and place it on one side of the Vault door in the Sewers, while interacting with the opposite side. This will open the door to the Vault, after which you will get the trophy.
  • Mind

Activate the door to the operation area

You will get this trophy after removing the Thread Bolt from the Reception and using it in the West Wing Corridor to access the Operation area.
  • Reattach a limb

Once you solve the W-2F West Wing Corridor combination key puzzle and gain access to the Intensive Care Corridor, you will meet the Priest for the fourth time in Room 2B. In the adjoining room, you will be able to access the alternate version of this corridor, where you will find an armless mannequin, as well as the Parchment Scroll, the Hammer and the Mannequin Arm in the adjoining rooms.

After obtaining these items, return to the reality through the mirror and solve W-2F Room 2D- Judge's Podium Puzzle (check the Additional Resources Suplementary Guide section for more information on how to solve this puzzle) and you will obtain the Staple Gun, with which, after adjusting it once, you will be able to "re"-attach the arm to the mannequin. Return to the reality one more time and proceed to talk with the Priest to get the trophy.
  • Locate Anna

After traveling to the past for the first time and solving the Skeleton Puzzle (check Journey trophy_bronze.png for more information), you will open a secret room inside which there will be two cells: an empty cell and a cell in which Anna will be. Simply interact with her to get the trophy.
