Make sure not to cut Santino's head before showing Murray to him. Guybrush will swap heads with him instead causing you to be unable to get the trophy.

Santino is NOT required. I screwed up the first time by putting the skull on him and locking myself out of the trophy. So the second time, I avoided him completely and the trophy popped.
