Step 1: Play through the game.
There are no trophies related to the game's difficulty level, so just pick Normal to speed things up. As you play through the game, work on completing as many kill trophies as possible. You should also get the Syringe trophies, the Asylum file and Patient file trophies, and the door unlock trophies. Once you've finished the game once, reload checkpoint 7-13 and pick the other door to get the other ending trophy.

Step 2: Cleanup run.
You should only be missing a few trophies after completing the game, if any.

-Weapon trophies

Once you've finished the game, you'll be able to use the Chapter Select feature to finish the remaining trophies. Use the trophy guide to find the best places to finish the trophies.

  • Acquired all trophies.

    Get all the other trophies.

  • Unlock first container.

    You'll find your first container in the first level, and you'll come across dozens of these throughout the game. To unlock a container, you will need to press each of the face buttons at the right time to stop the four wheels in the right place.

  • Kill first minion.

    See "Duress".

  • Kill 10 minions.

    See "Duress".

  • Kill 20 minions.

    You'll get this as you play through the game, and work on the weapon kills. If you're having trouble with the combat, rely on uppercuts (Hold L2 and press Square).

  • Used Health Syringe for the first time.

    See "I Can Stop Any Time".

  • Used 20 Health Syringes.

    There are dozens of these throughout the game. To use them, hold Square until the Syringe runs out or your health is full.

  • Idle for five minutes.

    Find a secure roomto wait in, and then just leave the game unpaused and idle for five minutes.

  • Find your first Asylum File.

    You'll find these scattered throughout the Asylum, and some of them are required reading to progress in the game. You'll only need to find one in the whole game.

  • Find your first Patient File.

    Just like with the Asylum files, you will find these scattered throughout the Asylum. You only need to find one in the whole game.

  • Nothing complicated about this, you'll die at some point either in a trap or from a minion.

  • Killed a Collared Minion with a collar detonation.

    Once you get an explosive collar attached to you, find a Collared Minion. You'll know one is nearby, because your collar will start beeping. Quickly distance yourself from the Collared Minion by closing and bolting doors or just knocking the minion down and/or running, and when his collar explodes, you'll get the trophy.

  • First kill with a Mannequin Arm.

    These are found throughout the game, but if you beat the game without finding one, use the Chapter Select to go to Obi's level, and in the room where you have to turn the steam off, climb the ladder and walk across the board, and you'll find a Mannequin Arm on a table.

  • First kill with a Bat.

    This is one of the most common weapons in the game, so it won't be difficult to find a Bat to use.

  • First kill with a Nail Bat.

    In the second level, you will come to a door that you have the option of barricading. Shortly after that, a minion with a Nail Bat will attack you. Kill him and take the weapon. Then it's just a matter of killing the next minion with it. There are a few other locations for the Nail Bat, so if you miss this one, don't worry about it.

  • First kill with a Mop Handle.

    This is another extremely common weapon. You should have no problem finding one somewhere in the Asylum.

  • First kill with a Pipe.

    When you get to the first minion that you're forced to fight, you'll be advised to arm yourself. There will be a pipe in a trash can for you to use. There will be lots of other opportunities for this trophy if you decide to kill the first minion with your bare hands.

  • First kill with a Scalpel.

    In Melissa Sing's level, when you get to the hallway where the guy shoots himself, open the door on the right, and then unlock the door on your right. You'll find a Scalpel stuck in a mannequin.

  • First kill with a Syringe.

    This is one of the rarer weapons in the game, but there are a few of them in Jeff's level. If you're still struggling, check this video, where they show you the location of a Syringe:

  • First kill with a Table Leg.

    In Jennings' level, the first minion you electrocute is carrying a Table Leg. Once he's dead, take the weapon and kill the next minion you find.

  • First kill with a Crutch.

    In the first unlockable room of the game, you'll find a Crutch. Use it on the next minion you find.

  • First kill with a Shovel.

    In Melissa's level, you'll enter a graveyard to find a specific grave. There are several Shovels leaning against the gravestones.

  • First kill with a Table Lamp.

    Right after you save Melissa, you'll find a Table Lamp in a nearby room. Should be easy to find.

  • First kill with a Hatchet.

    This is one of the more elusive weapons in the game. Immediately after saving Obi, or at the very beginning of Jeff's level, you'll find a hatchet sitting in a pool of blood right before you go up the stairs.

  • First kill with fire.

    There are a couple different ways to do this. The more obvious way is to find a Molotov Cocktail, and while you have a lighter equipped, light the Molotov and throw it at a minion to burn him to death, or you can try to get a minion who has a Molotov equipped to set himself on fire. Two possible options for this are outside in the courtyard where the minion is on the roof throwing Molotovs, or in the room where a Molotov equipped minion is behind a door. He'll keep throwing them at you even though the door is in the way, and he should eventually set himself on fire.

  • First kill with Scissors.

    In Obi's level, after hearing the audio tape about Obi, unlock the door nearby and take the scissors from the shelf.

  • First kill with a Gun.

    There are a few opportunities for this, but when you get to the hallway where the guy shoots himself, just take his gun and kill the next minion you see.

  • First kill with an Explosive Mine.

    You'll find the schematic for the Explosive Mine during the game. Once you've created an Explosive Mine on one of Jigsaw's workbenches, find a minion, punch him a few times to weaken him, and then select the Explosive Mine with R1, hold L2 to enter combat mode and hit R2 to drop the mine. Let the minion walk into the mine, and if he dies, you'll get the trophy, even if you die too.

  • First kill with a Stun Mine.

    You'll find the schematic for the Stun Mine during the game. Once you've created a Stun Mine on one of Jigsaw's workbenches, find a minion, punch him a few times to weaken him, and then select the Stun Mine with R1, hold L2 to enter combat mode and hit R2 to drop the mine. Let the minion walk into the mine, and if he dies, you'll get the trophy, even if you die too.

  • First kill with a Gas Mine.

    You'll find the schematic for the Gas Mine during the game. Once you've created a Gas Mine on one of Jigsaw's workbenches, find a minion, punch him a few times to weaken him, and then select the Gas Mine with R1, hold L2 to enter combat mode and hit R2 to drop the mine. Let the minion walk into the mine, and if he dies, you'll get the trophy, even if you die too.

  • Fought first NPC.

    You'll get this the same time as when you get the trophy for killing your first NPC, in Amanda's level.

  • Stomped first Minion to death.

    See "Merciless".

  • Stomped five Minions to death.

    This can be very difficult to get, mainly because it's hard to knock a minion down without killing him. Here's what I found to be the easiest way:

    - Start Amanda's level, and get to the checkpoint where you fight your first minion. When he runs into the room, hold to enter combat mode then hit him with an uppercut with . If he falls down, stand over him and you'll be prompted to hit a button. From there, you need to hit a few buttons at the right time. Get it right, and you'll stomp him to death.
    - If the minion dies instead of falling over, reload checkpoint Amanda 1-3 and try again.
    - After you get the first stomp trophy, continue through the level until the next checkpoint when you see a cutscene about your surgery. Once the game saves, load Amanda 1-3 again and repeat the process for your next Stomp kill. Continue until you have your 5 kills.

    I got this when Jigsaw had 3 dudes in cages. Right before you get to the big furnace room. You will know which one I'm talking about because there will be 3 cages, the one on the right will open first, then the left, then the middle cage will open last. Have a bat, and (hope for knockdown), quickly press to drop weapon and quickly perform curb stomp. You can get 2 curb stomps if the game is nice. Pause the game, change something in the settings (the game will save your progress), reload checkpoint! I was able to get it in 10-15 minutes that way!

  • Unlock 5 locked doors.

  • Unlock 20 locked doors.

    You should be able to get this through playing the game, but if you still need to find a few doors, there's a method similar to getting the Stomp trophy:

    -Play through Amanda's level until you get to the first door to unlock. Once you've unlocked it, continue through the level until you get to a checkpoint. After the game saves, reload Amanda 1-4, and unlock the door again. Continue to the checkpoint, and repeat until you get the trophy.

 

Secret trophies

  • Complete Reverse Bear Trap

    This is the first thing you do in the game. You'll see button commands on the side of the trap. Follow the instructions, and you'll get the trophy.

  • First Encounter With Pighead

    During the first level, climb the ladder. There will be a brief cutscene, and you'll get the trophy.

  • Release Amanda From Her Trap

    This is the first "boss" in the game. In the trap, there are two different chemicals that you need to direct; the Toxin needs to flow to one side, and the Antidote to the other. Watch the video below with help with the trap, but just keep in mind that every time one of the chemicals touches one of the valves, the valve will flip, aiming it in the opposite direction it was previously aiming. If you remember that the chemical will always change direction when it hits a valve, you shouldn't have any trouble.

  • Release Jennings from His Trap

    This is the second "boss" in the game. To free Jennings, you'll need to complete three of the same type of puzzles you have to do to open a Jigsaw box, where you add different sized gears to a board. Watch the video for help.

  • Release Melissa From Her Trap

    You'll need to complete several smaller puzzles by sliding a cube around a type of maze. Watch the video for more help.

  • Release Oswald From His Trap

    This is the third "boss" in the game. To free Oswald, you need to complete several of the fuse puzzles, by turning the different fuse pieces to make all the lights turn green. Watch the video for more help.

  • Release Obi From His Trap

    This is the fourth "boss" in the game. To save Obi, you need to complete three of the pipe puzzles that you have to complete throughout the game. They're a bit of a pain, so watch the video for more help.

  • Release Jeff From His Trap

    This is the fifth "boss" in the game, and the last person you have to save. To save Jeff, you need to complete a game of Memory, by matching pairs of items on the TV screens. Keep in mind the items you're trying to match are not identical, but they are things that go together, like a camera and film. Watch the video for more help.

  • Kill The Pighead

    This can't be missed. There are two possible ways to do this. One way is to lure Pighead into the electrified cage, lock him in and turn on the electricity, but the more straightforward way is to just uppercut the hell out of him. Every once in a while, you'll have to do a mini-game where you move the two analog sticks in different directions, and when that ends, just keep uppercutting him. Eventually he'll die, and you'll get the trophy.

  • Complete The Game Using The Freedom Door

    At the end of the game, you'll be given the choice of two doors. Choose the Freedom Door, and finish the game.

    Note: If you already picked the other door, reload checkpoint 7-13 and pick this door to get both endings in one playthrough.

  • Complete The Game Using The Truth Door

    At the end of the game, you'll be given the choice of two doors. Choose the Truth Door, and finish the game.
