Dragon Age: Inquisition isn't really a hard game to complete even though there's a trophy tied to the highest difficulty (Nightmare) in the game. Apart from that trophy and the trophy to beat the game on Hard (difficulty trophies stack), everything else can be done on whatever difficulty you prefer. If you're a Dragon Age or RPG Veteran, like a challenge or want to get the platinum as soon as possible, go for the one playthrough. If not, do it in two playthroughs. If going for one playthrough, be sure to read up on the countless missable and glitched trophies. Make sure to manually save often and don't constantly overwrite all your save files. Manual saves can get you most of the glitched trophies if any happen to glitch on you.
Start a new game on Casual/Normal if going for two playthroughs. Start on Nightmare when going for one playthrough.
Start a new game on Casual/Normal or Nightmare. In this playthrough, you mainly want to familiarize yourself with the game and want to focus on the missable trophies, getting most if not all of them out of the way. If you're not sure if you can do it on Nightmare immediately, you can always lower the difficulty if you feel like you're better of doing it in two playthroughs. Be aware that there's no New Game+ (NG+), so you won't be able to start a new playthrough with your existing character if you need to do a second playthrough.
WARNING: While playing on Nightmare, never press to "Reset" your settings in the option menu. Doing so will set the difficulty on Normal.
For the The Brightest of Their Age trophy, you'll need to recruit all companions. Most of them are obtained very early on in the game, except for two of them. Some of them are easily missed and you can also deny their request to join the inquisition. You can also decide later on to throw them out of the inquisition, which isn't a good idea if you haven't obtained the trophy before doing so.
Whenever you go back to Haven or Skyhold, talk to all your companions and advisors. They will give you Inner Circle quests. These are personal quests and they're required for the People Person trophy. You can do this at any time in the game, however, after completing all the Story missions all the Inner Circle quests will disappear. The same counts for the Beloved and Precious trophy, which requires you to romance someone. These also require certain quests to be completed and won't be available after the last mission. All missions you had active related to these two trophies become locked out. Make a manual save before doing the last story mission called "Doom Upon All The World", or finish this on your second playthrough.
Read up on the Persuasive trophy. This requires you to recruit 10 agents. There are about 22 agents in the game, but the majority can easily be missed by either not speaking to them as soon as possible or by ignoring their request to join the Inquisition. This trophy is also glitched.
There are also two missable trophies tied to 2 main story missions. During the "Wicked Eyes and Wicked Hearts" main mission, you can easily miss the Belle of the Ball trophy. Make sure to make a manual save before starting this quest. During the "What Pride Had Wrought", you need to make a certain choice at the very end. This is tied to the On Burning Wings trophy. If you want to avoid (minor) spoilers, make a manual save before starting or right after starting the quest. If you failed to choose the correct option, reload that save and try again. Or obtain this trophy on your second playthrough.
Throughout the game, you'll come across Herbs and Materials. Pick up as many as you can; you'll need many of them. Two trophies that require the most resources are the Quartermaster and the Master Alchemist trophy. Having to do all of this at the end might end up being a huge chore. Every time you go to a camp, talk to the requisition officer there if possible for a requisition. While requisitions for the Quartermaster trophy are pretty useless in itself, upgrading potions, tonics and grenades for the Master Alchemist trophy can make your playthrough easier.
Also be on the lookout for Ocularums, the sticks with a skull on them. They're represented as a skull on your (mini)map. Interact with them and you'll be able to find shards. Shards you've located will be placed with a marker on your map. After picking up one of these, you'll receive the Sharp-Eyed trophy. For the Keymaster trophy, however, you'll need to find all the shards in the game. Make sure to use any Ocularum you come across and pick up the shards they discover throughout the game.
And finally, close any rift you come across. Not only will you get experience, influence, power and more, but you'll need to close 75 Rifts in total for the Marked For Greatness trophy.
Make sure to read up on the three glitched trophies, Persuasive, Pathfinder, and Veteran. Make manual saves often and don't overwrite them constantly, as this can prevent you from having to play through the game more than you need to.
Start a second playthrough (on Nightmare) if needed.
If you didn't start out on Nightmare already, now is the time to do so. There's no New Game+ (NG+), so you won't be able to start a new playthrough with your existing character. Skip Hard if you need that trophy as well, since Nightmare will also unlock the Hard trophy. Nightmare isn't such a big difference from hard anyway. Lowering the difficulty at any point before finishing the story will void the trophy. Once you've got the trophy for completing the story on Nightmare, you can lower the difficulty to finish up any trophies. Manually save often and don't overwrite all your manual saves all the time. Now is also the time to go after any missable or glitched trophies if you've missed them on your first playthrough.

Introduction:
Jaws of Hakkon is a DLC for the popular game Dragon Age: Inquisition. It is single player only and it's difficulty will depend entirely on your game settings and advancement in the main game storyline. The developers have intended this DLC to be an end-game addition and as such they recommend a minimum level of 20. You can still achieve all trophies in this DLC with any in-game difficulty setting but it will be more difficult.
Personally, I picked this up after I was long done with the main game and so my character was as strong as I was able to achieve. My character was almost level 24 and I played this DLC with the Normal difficulty rating. With that context I gave a 2/10 rating for difficulty. A hike in game difficulty, lackluster equipment, and/or levels can drastically affect the difficulty you may experience with this DLC.
This DLC constitutes an all new sub-area within the World Map named Frostback Basin. It is an overgrown wilderness area in the southern mountains of Thedas, and is the home of the Avvar. While mentioned in the main storyline, this DLC brings us to the homeland of these fierce hunters and their culture. The storyline focuses mostly on historical investigation of the last Inquisitor who lived 800 years prior. As the story progresses your battles will be very much like the main game (same enemy class types) and eventually you will learn of some unfinished business of the former Inquistor. Handling this unfinished business is the conclusion of the DLC storyline.
If you are purely looking to bang out these 4 trophies in record time, then the price tag for this DLC is high as it should only take you between 3-6 hours (basically you run past all enemies where possible and sprint from quest to quest). However, this DLC does look amazing on the PS4 and the environment is incredibly realistic. There are many optional quests that you can take on, and many are similar to the main game (such as finding camps, staking landmarks, taking out rifts etc). If you take the time to do literally every quest and actually read all of the dialog options, journals, notes and books then the DLC price-tag does not seem too bad. If you take this route, the game should easily last you between 10-15 hours.
My personal take was this game just felt like one more (good) sub-area of the main game. It was more of the same (which again I felt was a good thing) without all of the normal DLC flash of "new items, new creatures, new loot, new skills, new whatever". So if you enjoyed the main game, you will definitely enjoy this DLC too.
Walkthrough:
The first question on my mind after installing a DLC is always, where is it?
- The first time you turn on the game after downloading the DLC, the game should blatantly inform you of a new War Table quest in the top right of your game screen.
- Go to the War Table in Skyhold
- Select the Ferelden side
- You will see Investigate Frostback Basin in the far lower left of the Ferelden area. This will require 8 power.
- After investigation the new Frostback Basin sub-area will be available on the World Map.
- You can choose to immediately "Venture Forth" from the War Table or use the World Map to enter the area.
Step One
Play through the DLC story.
Two of the four trophies are story related and cannot be missed. These two trophies are:

While playing through the DLC storyline there is one other trophy to keep a look-out for as it becomes possible once you enter the Old Temple. Refer to the trophy for more information (with spoilers if you wish):

Complete the optional "Guests of the Hold" quest-line.
This is an optional quest line that is available at the beginning of the DLC. While you can work through this quest line at any point, even before you do any of the main DLC storyline quests, you cannot actually complete this quest until you have finished all of the DLC main storyline quests. Refer to the trophy for more information (with spoilers if you wish):

I have seen it mentioned several times that the two non-storyline trophies are missable. From my investigation this does not seem to be accurate on the PS4. I was not able to find any way to permanently prevent the achieving of either of these two trophies.
With that said, these two trophies may be an area where you need to do some clean-up. If you are having difficulty I have provided extra information that should help you in the trophy descriptions.

The Descent is the second DLC for the popular game Dragon Age: Inquisition. It is single player only and its difficulty will depend entirely on your game settings and advancement in the main game storyline. The DLC can be completed any time after you get to Skyhold in the main game and have access to the War Table; it will scale to your current level. You can achieve all trophies in this DLC with any in-game difficulty setting.
Personally, I picked this up after I was long done with the main game and so my character was as strong as I was able to achieve. My character was level 26 and I played this DLC with the Normal difficulty rating. With that context I gave a 4/10 rating for difficulty. A hike in game difficulty, lackluster equipment, and/or levels can drastically affect the difficulty you may experience with this DLC. Compared to Jaws of Hakkon I found this DLC slightly more difficult.
This DLC constitutes an all new sub-area within the World Map named Deep Roads. If you are familiar with previous Dragon Age titles or Dwarf lore in general you should know what to expect here: an enormous collection of underground tunnels that used to be controlled by Dwarves but now houses Darkspawn. The storyline focuses on figuring out what is going on in the Deep Roads as the discovery could impact Lyrium production. Your battles will be very much like the main game (same enemy class types). The Deep Roads environment consists of six individual levels.
If you are purely looking to bang out these 4 trophies in record time, it should only take you between 2-3 hours (head to the quest marker as fast as possible with no detours). However, this DLC does look amazing on the PS4 and the environment is incredibly realistic. There are optional quests that you can take on, and two are similar to the main game (finding camps and staking landmarks). If you take the time to do literally every quest, and actually read all of the dialogs options, journals, notes, and books then the DLC should easily last you between 6-8 hours.

To access the DLC:
- The first time you turn on the game after downloading the DLC, the game should blatantly inform you of a new War Table quest in the top right of your game screen.
- Go to the War Table in Skyhold.
- Select the Ferelden side.
- You will see Disaster in the Deep Roads in the upper middle of the Ferelden area; the map icon is unique in that the black marker has blue Lyrium on its sides. It does require 16 power.

It should be noted that the primary optional quest revolves around finding Bloodstained Ancient Gears. While 3 gears are required for the first door on sub-level 2 for the main story quest, all of the rest of the gears are used for the optional Sacrificial Gates of Segrummar quest. There are several dozen of these gears with some in plain sight and some in tricky hidden places. Interaction with the Expedition Table is necessary for this optional quest in order to reach some gates and gears; the Expedition Table requests are primarily quests that cause paths to be built to access certain areas (bridges or cleared rocks). The primary purpose of this optional quest is lots of loot and some decent experience.
Warning: While these DLC trophies can be completed on any difficulty, changing your difficulty level here will also affect your main game trophies (if you have not yet achieved your main game difficulty trophies).

Trespasser is the third and final story-related DLC addition to Dragon Age: Inquisition. It is set two years after the end of DA:I, and quite nicely explains what your followers have been doing in this time. The new content is meant to be a post-game addition, and so your character is recommended to be at minimum level 20. The enemies in the new content are harder and more challenging, especially at higher difficulty levels so because of that, I would not suggest completing it on Nightmare.
It's also the utter end of the game for your character – you will decide what you wish to do with the Inquisition and your choices will affect your ending. But before you do, you will have the ability to speak to your companions once again and learn what they have been up to in the last two years, as well as what their plans and opinions are for their and the Inquisition’s future. In my opinion, the story section of the DLC is well-done and satisfying.
This DLC's trophies are a bit different than usual. They include two sections of trophies, the first includes 3 trophies that relate to the new DLC content. The second, however, includes 8 trophies that will have you replaying the original, base game content but using "Trials". These trials are modifiers meant to handicap you in some way or another, and come with a possibility of rewards in the form of equipment or sigils. I personally suggest that you do the DLC story content before returning to the original game to do the Trials.
IMPORTANT TIP: In both the new and old content, you should save often and make multiple saves. Firstly, almost all of these trophies are missable so it's always good to have multiple back-ups just in case and secondly, even BioWare has suggested this as it seems the trials can be a bit buggy. Their warning states: "In rare cases, trials may cause enemies or followers to behave unpredictably. Keeping multiple save files is recommended". Finally, Trespasser can quickly become buggy, and if that happens you'd need to revert to a manual save to fix it. Sometimes it glitches where you cannot loot and cannot move after engaging in combat.
The trophies are split into two sections: Trespasser DLC content, and Base-game Content (in other words, the original content from Dragon Age: Inquisition, the main game).
As aforementioned, I would suggest doing the DLC content before you tackle the Trials. Firstly (and most importantly), so that you are better able to enjoy the ending of your story. Additionally, another good, more tangible reason to do the DLC first is that when you locate the armor pieces from the Coroner puzzles, you are also rewarded with the schematics for those pieces, which will be available via the Golden Nug and thus can help you when you go to do the Trials in a different playthrough.
Play through the DLC content.
WARNING: Once you begin Trespasser, you can no longer return to Skyhold or any other area, in fact. It is the beginning of the end and once you've completed this step, you will no longer be able to play this character so make sure you're completely ready: synchronize everything with the new Golden Nug in the Undercroft, finish up any side-quests or previous DLC content you want. I also advise making a save prior to starting the DLC. That way, once you’ve finished, if you want to go back and keep playing, you’ll be able to.
When you are ready to begin, simply go to the War Room. On the Orlais side, there should now be a new mission to explore for 8 Power, which will take you to the Winter Palace, which will replace Skyhold as your hub.
By the end of this step, you should earn the sole story-related, non-missable trophy:

During this step, you should also keep track of the two collectible-related trophies, as there is a point of no return and they are easily missed:

Lateral Thinker
Begin a new playthrough and/or load earlier saves, if you have them.
There are 8 trophies that require playing through the main game again, using specific modifiers. These modifiers can be found by: Opening the main wheel using the options button . Select options, then select Trials (right under Gameplay). Here you can toggle all the trials on and off. All trials are meant to make certain situations more challenging but also increase your chances of receiving "special shipment" rewards, which you can find in the Undercroft in Skyhold.
WARNING: While in this step, never press to "Reset" your settings in the Options menu. Doing so will deactivate all trials and void any in-progress Trials trophies.
If you happen to have early saves from the game, that should work for most of them. You'll need saves that are, at minimum, at the following spots:
Trial of the Magician: Prior to beginning either In Hushed Whispers or Champions of the Just.
Trial of Temperance: Prior to entering Emprise du Lion at all.
Trial of the Empress: Prior to killing the level 12 Ferelden Frostback dragon in the Hinterlands.
Trial of the Tower: Prior to killing the level 19 Hivernal dragon in Emprise du Lion.
There are two trophies that can be done at any point in the game, even on a character that has already completed it. These are:

Trial of the Emperor
And finally, there are two trophies that require an absolutely fresh playthrough and cannot be achieved using previous saves:
Trial of the Lovers
Trial of the Fool
If you do not have saves that match the requirements, it is easily possible to do all of the Trials trophies in one new playthrough. However, I do suggest doing most of these on Normal difficulty because Nightmare will make them more challenging so it's best if you don't complete these on your Nightmare run if you have yet to get the trophy for it. Additionally, having a character which has already completed the game will make this step much easier because you will have access to all of the schematics you've found via the Golden Nug.
