Walkthrough:
Step 1: Complete the game on Apocalyptic difficulty
It is important to note that you can't continue playing the game if you beat the final boss. Furthermore, there is no "continue" or "New Game+" option to allow you to finish up collecting things from your first playthrough. Because of this you should make a save prior to the final boss, allowing you to load up your first run in order to gather all of the collectibles. If you want to know exactly where to save the game to ensure that you won't beat the game without saving, read the following:
Save After Forging Ultimate Blade - After you forge the Armageddon Blade for the "Ultimate Blade" trophy, you should save the game in a slot that you will not overwrite with a new game save as the very next thing you will do is fight the final boss and the game will be over.
If you want to travel to Vulgrim in the Scalding Gallow before doing so, that is fine too, because you will be doing a lot of traveling around that location.
Unless you are opposed to playing on a max difficulty, or do not mind playing through the game twice, I suggest you begin the game on Apocalyptic. It is not very challenging, and it actually becomes easier the farther into the game you get.
During your first playthrough your primary focus should be on completing the four missable trophies (Open Air Parking, High Flier, Aerial Predator, Who's Counting?), and getting the Fury's Embrace enhancement (see Tip 2 below on how to get it). The missable trophies and their events occur in the order I just listed, so be prepared to do them as such. If you do not wish to stress about them during your first playthrough of the game, simply save at their locations (as shown in the trophy descriptions) and finish them at a later time. Just be sure not to save over those save files.
Aside from the four missable trophies you should simply enjoy the game, and not fixate on the trophies. Most of the non-story related trophies require you to backtrack, or expend a lot of time doing a repetitive task. You can't effectively backtrack until you've attained all of the required tools to complete the puzzles, which you will not have until the last two hours of the game.
Do not stress over collectibles, collecting souls, or maxing out your weapons but don't purposely avoid collecting ones that you come across either. Just take note of which ones you've collected by checking them off this list. The same goes for Health and Wrath chests.
Step 2: Collect all the collectibles
If you have not yet collected the "Fury's Embrace Legendary Enhancement" you should immediately refer to "Tip 2" below in the "Tips and Tricks" section before reading further, because I will be assuming that is the case from here on out.
Before you start out on your search I suggest visiting the Collectibles Locations portion of this guide (click the link to open in a new window), and print it out. You should refer to it as a checklist, and literally check off each one after you have collected it and explored the area. That way you will know which areas have been covered, removing the need to double check those areas.
You should begin your collectibles search at "The Twilight Cathedral" and work your way out from there.
You will have unlocked the following four collectible trophies by the end of this step: Legendary Form, Wrath of War, Full Power, and World Raider.
Step 3: Cleanup
The key trophies that will remain at this point will be Battle Hardened, Dark Rider, and quite possibly Improvised Kills and Horseman. Refer to those trophy descriptions in the guide below for how to achieve these trophies.
Note by this step if you are missing any story related trophies labeled as "Impossible to miss" in this guide, your game has bugged and you will have to do that step again.
If you haven't already completed the 4 missable trophies (Open Air Parking, High Flier, Aerial Predator, Who's Counting?) I suggest loading up the saves that you should have previously made and obtaining them now. If you didn't make saves at the points needed for those trophies you will have to start an entirely new playthrough.
For every other trophy that doesn't fall into the category of story, collectible, or missable, refer to trophy guide below on how to acquire the trophy as quickly and as painlessly as possible.
Tips and Tricks:
The following tips aren't required to do in a playthrough by any stretch. However several of the trophies, as well as the game as a whole, are made easier due to these tips and therefore are referenced in several of the trophy descriptions.
Tip 1 - Iron Canopy Spiders - Two trophies (Devastator and Battle Hardened for weapon EXP farming) are easily unlocked by utilizing the spiders found in the Iron Canopy (Silitha's level) so it is crucial that you know how to make your way to the proper location, and how to respawn the room.
- First use the Serpent Hole to Ashlands.
- Travel back to Silitha's Lair.
- Backtrack back to the large cobweb door that was sealed with the 5 spider symbols.
- Look away from the cobweb door and go into the small tunnel just to the left. You should see some demonic growth inside it.
- Jump down and inside that first room you should run into 17 mini spiders.
- Do whatever it is you're there to do (Devastator, or weapon leveling for Battle Hardened).
- Go back up to the wooden planks near the cobweb door and the spiders, as well as the environmental items, will respawn.
- Repeat aforementioned steps.

Tip 2 - Fury's Embrace Legendary Enhancement - Once you reach The Black Throne you will be able to get this enhancement. It reveals all collectibles on the maps, as well as fully fleshing out the maps.
To the far right of Azrael is a path that leads up to the top of the tower. You'll reach one of the materializing bridges/paths. To the left and up is a chest, and next to it is void panel. Use the Voidwalker on the panel. At the very start of the room is another void panel. Use this panel to get to the chest.
