Use the Precision ability to rack up three head shots in rapid succession.
You won’t unlock the Precision power until later on in the story, by competing “Good Gets Better”. Once you’ve completed it, you can access Precision at anytime by pressing on the D-pad.
As described in the trophy, your goal is to do three successful headshots in rapid succession. By that, the “Amazing! 3-in-1 Blow” stunt bonus must show up on screen. Rather than attempt this during the story or a side mission, try to instead look for militants in groups of four patrolling the streets when you're not in a mission. Once you’ve found one, head to a nearby rooftop without gaining their attention, or engage them Rambo style at the risk of sustaining heavy damage, and take aim.
Place three well aimed headshots, and the trophy will unlock.
Ohh I got a good tip here, when going on your Evil side try having Nix use her blob technique on to a bunch of enemies, you know where Nix holds of like 3 - 4 enemies with her powers, use the Precision ability there since she's holding them off it's easier for you to shoot them, neat
In flood town, you'll encounter enemies that rise up on ice towers and shoot an ice ray at you. All you have to do is shoot them in the head 3 straight times. They don't rect to the head shots, so you don't even have to adjust your aim.
Comment #5 is right on. I was having a really hard time with this one, and accidentally discovered I could shoot one of these ice tower guys three times straight and the trophy popped. Maybe the guide should be changed to reflect this. It is SO much easier than trying to mimic the video!
A mission early in the game where it is easy to get this (without having to wait for the precision power to by upgraded, which is a prerequisite for most of the UGC missions for this trophy) is the side mission "Triage" on the first island. There will be at least 7 militia guards escorting 6 hostages. There are two guards at the front and in the middle and 3 trailing behind the group. It is easy enough to sneak up behind them and precision head shock each of the three at the back of the pack. Make sure to have a few extra charge cores just in case you miss. I had to restart the mission only once before I was able to do it right.
This was easy on the Flood Town mission "The Dunbar Beam". While on the spotlight platform, chose to headshot one of the monsters instead...they usually just stand there and you can do a precise headshot three times in a row on one of the creatures.
