ZOLANTON
2 ways 1) Go to every city and check the maps VERY CAREFULLY. 2) Follow this video [ame=http://www.youtube.com/watch?v=praWH2Y6ECc]YouTube - Assassins Creed 2: Codex Pages (Part 1)[/ame]
DPsx7
Just went through that myself since I missed exploring the top of Venice before progressing. Just visit the other areas with the quick travel and look for missed viewpoints which should point you to the missing piece.
andy_505-
Go to every city. You will find them.
Phodda
They appear in your map. If I'm not mistaken, Uncle Mario gives you a map with the location of all codex pages (assuming you didn't collect them early). If don't, just check your city maps. But the question remains: Did you synced all viewpoints?

lealea25
you will have to probably buy map or whatever it is called from the painting shop, otherwise they won't appear
justmiike
I did this yesterday, after sync a viewpoint I was able to see a codex page I was missing. So I can confirm that by syncing, they do reveal the location.
